Improved quality of life, independence, and choice.

Liberty has provided services to aging individuals and adults with disabilities since the late 1980s. Our services include medical and behavioral staffing and supports, home-based needs assessments, and programs for adults with neurodegenerative conditions and traumatic brain injury. Our clinical teams stabilize acute conditions, facilitate the development of self-management skills, and help people maintain stable, productive, and satisfying lives in the community.

Liberty provides medical leadership and on-site clinical staffing for long-term care facilities, implements acute and long-term geropsychiatry treatment programs, and manages statewide assessments for long-term supports and services. We have also provided statewide quality oversight for thousands of citizens who are elderly and have one or more disabilities.
